International Health Certificate Agreement

Regulations for a pet traveling outside of the continental US is governed by the nation the pet is traveling to. Those regulations and requirements may change at any time and without notice. The requirements and forms may be complex and may not be readily available to us. It is the responsibility of the pet owner to obtain written documentation of the requirements needed for the county of travel. These documents may be obtained from the embassy or consulate of the county of destination and may be available on the internet (www.aphis.usda.gov). Vaccination requirements, microchip requirements, and rabies titer requirements are especially important.

If proper requirements are not completed and/or documents are improperly completed, the pet may be subject to additional costs including but not limited to additional treatments and/or a quarantine period as required by the county of destination.

I understand that the regulations for travel for my pet are regulated by the country or countries my pet may visit.
The costs associated with any additional treatment and/or quarantine, and/or the cost associated with the refusal for the pet to enter the country of destination in solely my responsibility as the owner.
We take our guidance from the United State Department of Agriculture Animal and Plant Health Inspection Service (USDA, APHIS). However, ultimately it is the pet owners responsibility to insure all requirements have been completed correctly and in a timely manner.

We make no claim of warranty, suitability or acceptability for international health certificates beyond our endorsement on the forms provided by the owner and/or USDA-AAPHIS office in Salt Lake City or USDA-AAPHIS website. We do not warranty in any way that the pet will not be quarantined.

It is strongly recommended that you contact a professional transport company to assist with the export requirements for your pet's travels.

Work with your veterinarian to complete all required paperwork. Please consult our APHIS pet travel website to access the required certificate/form, when available.

After your health certificate has been completed, signed, and dated by your veterinarian, most countries require that international health certificates be submitted to a USDA APHIS Veterinary Services office for endorsement. Some offices offer to review health certificates prior to submission.

Please work with your endorsing USDA office to determine your options for paperwork review and/or submission.

Address of location pet will be staying and contact information of responsible party – may or may not be the owner if someone else will be accompanying pet.

Pet Information:

Be aware most countries require microchips to be in place prior to travel, some even require microchip to be in place prior to vaccinating – this timeline is critical to your travel ability. Please contact the USDA for your country’s requirements.

Vaccine History:

We must have hard copies of vaccination history and Rabies Certificate at least 24 hours prior to Health Certificate appointment if vaccines were not done at Wasatch Animal Hospital. Any vaccines not current or without proper documentation will need to be updated based on destination requirements before the health certificate can be issued.
Obtaining vaccine titers is a process that can span weeks or months. This timeline may interfere with travel plans and must be researched thoroughly.

Parasite Prevention:

Many countries recommend specific products be used at pre-determined timelines, and may require application or administrations of medications to be supervised by a veterinarian. Please be sure to contact the USDA for specific guidelines based on your travel plans.

They were able to squeeze my dog in last minute for an emergency appointment. They offered not only compassion but multiple treatment options to help him. They took care of us and made sure to fix our emergency issue quickly. We came in at closing time on Tuesday and had an appt for the next morning right away. They did all her bloodwork the night before so she could go straight into operation. Our girl is 4 days out from her surgery (multiple tooth extraction) and doing great.

We recently switch over to Wasatch Hollow from another vet in the area. We have a diabetic dog who just had cataract surgery and one with severe allergies. Both of our dogs seen DR. McClellan. He was very good with both of them and answered all of our questions. They accepted all of the medical records from the former vet which made getting medication refills quickly. They did not require to repeat tests that were just done on both dogs. I would highly recommend this place.

I had to go here for the hardest thing a pet owner could do which was have my dog put down. They have shown such compassion and sympathy unlike any I've seen from a vet. Thank you guys for helping me through such a hard time.

Great experience for my cat Mango. Dr. Blaine and nurses were very friendly and thorough. She reassured that my cat seemed healthy but also explained further procedures that could be looked into. They also called the next day to ask if I had follow up questions. Would recommend!

Absolutely incredible staff. This was my first vet appointment in a long time, and my first appointment with Wasatch Hallow. The staff was incredibly nice and friendly and helpful. Dr. Blain was very nice and smart and helped address my concerns with my dog Chip, and cat Arthur. My nurse (I didn’t catch her name) was incredibly transparent about recommendations for my furry family, and was able to give me costs for all the things I needed to protect my loved ones and updated my totals as we went along .
